Scheduling An Appointment

To schedule an appointment, physically active individuals may contact The Sports Medicine Center directly. For individuals who are under a managed care program, referral must be made by his or her family doctor. For athletes involved in organized sports teams, referrals may come through coaches, athletic trainers, school nurses, team doctors and emergency rooms. When you call to make a referral or schedule an appointment, we have a few important questions to ask you. Having the following information ready when you call will help us schedule more efficiently:

• The athlete's name.
• Age.
• School or college.
• Sport.
• Site of injury (please be specific).
• X-rays, MRI, bone scan, if taken (at what facility).
• Specific Symptoms.
• Athlete's telephone number.
• Name of referring individual (coach, physician, etc.).
• Type of insurance (please bring your card the day of the visit).
• Social security number.

Athletes under 18 must be accompanied by a parent or legal guardian, or bring a note giving us permission to provide care.
