Karen Clark, M.S., C.R.N.P.

Nurse Practitioner-Gerontology

Karen is the Nurse practitioner for the SH palliative Care Program. In this role she provides primary medical care to the patients referred to the service with a focus on determining care wishes, disease process education and providing comfort, in collaboration with the patient's primary care doctor. She develops policies and procedures with the Palliative Care Team that promote patient and family autonomy.

Pat Kiernan, M.S.W.

Education:
Bachelor of Arts in Psychology, Penn State University
Masters degree in Social work, Marywood University

Pat serves as the medical social worker for the Palliative Care Program. She works with patients and their families to help coordinate the resources available to them throughout the Health System and the community, as well as address the psychological issues and questions that are important to patients. Pat has worked for 15 years in the home care and hospice environment, and 10 years in behavioral health.
