Skilled Nursing

Home care nurses help patients achieve medical independence in the comfort of their own homes. A nurse makes visits to assess, teach and monitor patient conditions, and then communicates this information back to the patient's physician as needed. Some of the services provided by our nursing staff include:

  • Skilled observation and assessment.
  • IV therapy.
  • Blood and specimen collection. .
  • Health education.
  • Wound care.
  • Numerous other nursing services as ordered by your physician.
  • Telehealth Home Monitoring - Honeywell HommMed™ Monitor.

Specialty Programs

Rehabilitation

Our staff of rehabilitation nurses, aides, physical therapists, occupational therapists and speech therapists provide services to help patients become more independent in their home

  • Physical Therapy - Physical therapists help adults and children regain their strength and abilities after an illness, injury or surgery.
  • Occupational Therapy - Occupational therapists help patients restore their independence with daily living skills such as bathing, dressing, preparing meals and using home medical equipment.
  • Speech Therapy - Speech therapists assist patients with swallowing and communication disorders to help them improve their skills.

Bridge Program

This program is for patients with a life limiting illness or terminal disease. Nurses work with patients who are still receiving aggressive disease treatment and are not yet ready to embrace the Hospice philosophy.

Maternal Child Health Services

Pediatric Services

Specially trained Registered Nurses work in the home with high risk newborns or children with serious illnesses. Every effort is made by our team of professionals to provide parents with education and support as they care for their children.

Children that can benefit from this program include:

  • Premature and other high risk newborns who require complex home care treatments such as bili blankets, apnea monitors, oxygen therapy, tube feedings, nebulizer treatments and multiple medication and administration.
  • Children with cardiac defects, asthma, bronchiolitis, pneumonia, cystic fibrosis or other respiratory problems who need cardiopulmonary assessment and instruction from experienced maternal/child nurses.
  • Children with special nutritional needs such as those with diabetes, cleft palate, Crohn's disease, infants who have difficulty gaining weight and failure to thrive infants.
  • Children with cancer and AIDS. We offer parents emotional support, instruction on prevention of infection assistance with complex treatment regimens, such as chemotherapy in the home setting.
  • Children who have had surgery and whose parents need instruction in IV antibiotic therapy and care of an incision.

Services For New Or Expectant Mothers

The pregnancy and postpartum times are stressful for women. And since most mothers go home within 24-48 hours after delivery, they don't have much time to think about their own feelings. Our registered nurses are specially trained to work with new and expectant mothers. They provide support for these women who may be experiencing the various emotions that come with being a new mother.

Upon physician referral, Maternal/Child Health nurses provide:

  • Skilled nursing assessment for high risk pregnancies such as gestational diabetes, preeclampsia, IV hydration antibiotics, medication administration and wound care.
  • Preterm labor education and monitoring as well as assessment.
  • Non stress testing.
  • Emotional support along with postpartum education.

Healthy Beginnings Plus

Pregnant women receiving care from The Williamsport Hospital & Medical Center Prenatal Center, Williamsport OB/Gyn and The Williamsport Hospital & Medical Center’s Family Practice Group, who receive Medical Assistance, are eligible for a unique program called Healthy Beginnings Plus. The mother-to-be receives home visits from nurses who provide information and education that helps ensure a healthy pregnancy and baby. Home visits are provided for up to eight weeks after the delivery of the baby.

Services For First-time Mothers

If you are expecting your first baby and meet certain income guidelines, you may be eligible for a little extra help at no cost to you. Susquehanna Home Care & Hospice provides the Nurse-Family Partnership to pregnant first-time mothers. Maternal child nurses visit mothers during their pregnancy up until the child's second birthday. These specially trained Registered Nurses provide education and support as they care for new mothers and their babies in their homes. Personal Care Services

Our personal care staff help individuals who wish to remain in their home environment and need assistance with personal care and tasks for everyday living. Our staff is competent and caring and provides assistance tailored to meet patients' needs.

The home health aide provides care under the direction of an RN such as changing simple dressings and transfers in and out of bed, chair or bathtub; personal care such as hair washing and all personal hygiene needs, assisting with exercises, and the monitoring of self-administered drugs. The home health aid also provides light housekeeping such as doing laundry and dishes, meal preparation, errands and grocery shopping.

Medical Social Services

Social workers compliment home care or Hospice services by offering assistance in many areas, such as:

  • Coordinating community resources to help individuals remain as independent as possible in their homes.
  • Assisting patients and families in coping with life changes that illness brings.
  • Counseling for those suffering from terminal illnesses.

Infusion Services

Some patients need to receive medication or nutrients through a vein, under the skin or in the spine. Our comprehensive service allows patients to receive this therapy in their home. Pharmacy staff work with home care nurses to provide teaching, nutrition support, pharmaceuticals and supplies.

Home Monitoring Equipment

Susquehanna Home Care & Hospice offers a unique service to patients in their homes - the HomMed™ monitoring program. This daily home monitoring equipment is a small machine placed in a patient's home that collects an individual's vital signs such as weight, blood pressure, oxygen saturation, heart rate and temperature.

Each day a calming voice guides the patient through a simple procedure to collect this data and then sends this information to nurses at Susquehanna Home Care & Hospice for evaluation.

Nurses notify the patient's physician if there are any changes in condition so problems may be addressed early - preventing an emergency room visit or hospital admission.
