Pharmacy staff work with home care nurses to provide teaching, nutrition support, pharmaceuticals and supplies. Home Monitoring Equipment
Susquehanna Home Care & Hospice offers a unique service to patients in their homes - the HomMed™ monitoring program. This daily home monitoring equipment is a small machine placed in a patient's home that collects an individual's vital signs such as weight, blood pressure, oxygen saturation, heart rate and temperature. Each day a calming voice guides the patient through a simple procedure to collect this data and then sends this information to nurses at Susquehanna Home Care & Hospice for evaluation. Nurses notify the patient's physician if there are any changes in condition so problems may be addressed early - preventing an emergency room visit or hospital admission. Patients have a sense of security and independence with HomMed™ in their homes! |
