A Special Place

The Gatehouse

In the wall that divides
This world from the next
There's a gap and a gate
With a gatehouse before it
And for each one who passes
From here "over there"
There's a time in the gatehouse
Just to be and prepare.
Sometimes seconds, or hours
Sometimes days, weeks, or months
Even years in the shadows
Of the gatehouse to wait.

As I train as a healer
And then work through the years
I have calls to the gatehouse
And I often go in.
But the unspoken words
Of past teachers and friends
Speak within me
And unsettle me.
"It's a dark, place, it is hopeless,
it's depressing and sad,
Do your business, and keep your distance,
Do your task and escape!
"But I cannot escape,
I'm drawn back, I am called
To the soft light inside
And to what it reveals.

In the sadness there is laughter
In the suffering there is hope
In the hurt there is healing
In the loneliness, love
And for me if I listen
And I learn with my heart
There is wisdom
Deep truth
And a glimpse of my God.

Now I work in the gatehouse
I go help there each day
And I know, just for certain,
That one day I will stay
It will be my turn then
One cot will be mine.
And I wonder when that time comes
As I lie there and wait
Will the healer beside me
Feel at home by the gate?

Susquehanna Home Care & Hospice recognizes that home is the best place for Hospice patients. However, remaining at home isn't always possible. That is why we opened The Gatehouse, a seven-bed Unit on the Divine Providence Hospital Campus.

By providing a peaceful, home-like environment, The Gatehouse makes a difficult time easier for both the patient and his or her loved ones. Families can spend quality time with patients in private bedroom suites, a comfortable family room, a kitchen and a meditation room-all while patients receive around-the-clock, skilled nursing care to manage their difficult symptoms.

The Gatehouse is available to patients who need specialized pain and symptom management, who are experiencing a family crisis or who are actively dying. Stays in The Gatehouse will range from 3-10 days, depending on each patient's needs and health status.

Admission Criteria

Pain and Symptom Management

The patient must require skilled medical or nursing care to manage intractable symptoms that cannot be managed safely or effectively at home, such as: pain, nausea or vomiting, respiratory distress, seizures, severe agitation or other physical or psychological symptoms. The goal is to improve symptoms and prepare the patient and the family/caregiver for the patient's return to home.

Family Crisis Management

A terminally ill patient may need skilled nursing care if the family support system is unable to properly care for the patient at home. The goal is to provide appropriate care and support to the patient and family/caregiver until the patient can return home or to an appropriate facility.

Imminently Dying

The actively dying patient is one whose death is imminent (hours to days). The goal is to provide quality End-of-Life care to the patient and support to the family or caregiver. If a patient's condition improves the patient may be discharged to the home or appropriate facility.

Alexander Nesbitt, M.D., Hospice medical director, brings a great deal of compassion, training and expertise to the Hospice Program. He has been with Hospice since 2000 and is the only physician in the area who is Board Certified in Hospice and Palliative Medicine. Dr. Nesbitt works full-time overseeing Hospice care in patients' homes and in the inpatient Hospice unit, The Gatehouse. He has dedicated his life to providing much needed care to patients at the end of their lives.
